An appointment for a clinical interview with the ADHD Consultant-Nurse Practitioner.
During your video appointment, the ADHD Consultant- Nurse Practitioner will review your Assessment Tool and talk to you about a diagnosis and treatment plan.
Your consultation report will be reviewed by our ADHD Team of healthcare professionals.
You will receive your consultation report by email. Reports can be expedited if needed sooner.
*The report includes: a clinical diagnosis; accommodations for work/school, non-medication and medication management recommendations; access to a 1-hour group sleep consultation and personalized sleep plan.
You can share the report with your family physician/nurse practitioner to start treatment. The report serves as an official medical documentation of ADHD diagnosis and recommendations.
