You will receive an Assessment Tool by email within 48 hours.
Complete and return the Assessment Tool to the ADHD Centre.
An appointment for a clinical interview with the ADHD Consultant-Nurse Practitioner will be arranged usually within weeks of returning your completed Assessment Tool.
During your video appointment, the ADHD Consultant- Nurse Practitioner will review your Assessment Tool and talk to you about a diagnosis and treatment plan.
Your consultation report will be reviewed by our ADHD Team of healthcare professionals.
You will receive your consultation report by email within 4 – 6 weeks. Reports can be expedited if needed sooner.
*The report includes: a clinical diagnosis; accommodations for work/school, non-medication and medication management recommendations; access to a 1-hour group sleep consultation and personalized sleep plan.
You can share the report with your family physician/nurse practitioner to start treatment. The report serves as an official medical documentation of ADHD diagnosis and recommendations.
