By qualifying for the Disability Tax Credit (DTC), you may receive refunds on past taxes and reduce future tax payments, providing financial relief. This can also open doors to additional support, such as the Registered Disability Savings Plan, Canada Worker’s Benefit, and Child Disability Benefit—resources that can help ease the financial burden for you and your family, allowing you to focus on your well-being.

1. Applying for the credit –

There are 2 sections to the  form. One section is completed by you and the medical portion can be completed by a physician, Nurse Practitioner, or psychologist. If you do not have a physician, the ADHD Centre for Women can provide assistance through one of our physicians.

2. Claiming the credit on your tax return

If your application is approved, you may then claim the disability amount on your tax return.

Make a Payment

Make payment online for $350.00 which is the ADHD Centre for Women’s fee to complete the Disability Tax Credit Application Form.

You will be contacted by email within 2-3 days with further information.

Receive

The Disability Tax Credit Application, completed and signed by one of our physicians will be emailed to you. We only complete the pdf/paper version of the Disability Tax Credit application form, not the online application form.

Submit

After you complete your section of the Disability Tax Credit Application, you may submit it to your accountant or directly to the Canada Revenue Agency (CRA).

The Disability Tax Credit Application, completed and signed by the Medical Director, will be emailed to you. The Medical Director only completes the pdf/paper version of the Disability Tax Credit application form, not the online application form.

After you complete your section of the Disability Tax Credit Application, you may submit it to your accountant or directly to the Canada Revenue Agency (CRA).

While we cannot guarantee what will happen in any particular person’s situation, our experience is that patients with confirmed ADHD have received thousands of dollars in past taxes refunded to them, as well as reduced tax payments in future years.
